Acne
Mild: Benzoyl peroxide (2.5%) + adapalene (0.1%) Moderate: Add prescription topical antibiotic (clindamycin) Severe: Oral medications (isotretinoin) with clinical supervision
Key principles:
- Benzoyl peroxide kills bacteria without resistance
- Retinoids prevent new comedones from forming
- Don't pick — increases scarring risk by 10x
Hyperpigmentation
Post-inflammatory hyperpigmentation (PIH) and melasma require patience:
- Vitamin C — prevents new pigment formation
- Niacinamide — inhibits melanin transfer
- Azelaic acid — prescription-strength brightening
- Hydroquinone — most effective depigmenting agent (use under supervision)
- Sunscreen — critical to prevent darkening

Rosacea
Rosacea requires a different approach than typical skincare:
- Avoid triggers — alcohol, spicy food, extreme temperatures
- Gentle products only — no fragrance, no harsh actives
- Azelaic acid 15% — reduces redness and inflammation
- Metronidazole — prescription topical for papules
- Laser/IPL — for persistent redness and visible vessels

Seek clinical evaluation if:
- OTC treatments aren't working after 8 weeks
- You have cystic or nodular acne
- Skin changes are sudden or asymmetric
